Transcript Information

Instructions
If written request, request must include Name, name while attending PNCA (if different from present name), Social Security Number, Dates of attendance, program, address(s) where transcript is to be sent, number of copies, and signature. Official transcripts If issued directly to student will be marked “Issued to Student”. Transcript paper is tamper proof and “copy” will appear when trascript is photocopied. Faxed transcripts are not considered official. Hand delivered transcripts will be accepted as official if they are in a sealed envelope.

Transcript Cost:
There is no cost for transcripts.
