Transcript Information

Instructions
Transcript request forms are available on the Web at http://www.pdx.edu/media/t/r/transcript_request.pdf. Written requests are also welcome and must include: full name and any previous names, current mailing address and telephone number, student ID/SSN, approximate dates of attendance, date of birth, date and type of PSU degree earned (if any), complete address where transcript(s) should be mailed, the number of transcripts being requested, and the student’s signature. Unsigned requests are not processed. Official transcripts have the signature of the Director of Admissions, Registration and Records and the school seal printed at the bottom right corner. Each official transcript is sealed in its own envelope. The transcript has a light green security border and the school seal on it. Faxed transcripts are considered official when accompanied by a cover sheet following the guidelines recommended by the AACRAO Fax Guidelines Task Force.

Transcript Cost: $4 per copy; Rush Fee (for processing within 24 hours of receipt of request): $10; Express Delivery (includes rush fee): $25.
