Transcript Information

Instructions
“Transcript request form-not yet on web (soon). If written, request must include: name, name while attending WS, mailing address, social security number, address to send transcript, number of copies, signature, payment. Official transcripts are stamped across the envelope seal and have the school’s seal embossed on the transcript itself. Transcript paper is tamper proof and “”copy”” will appear when the transcript is copied. Faxed transcripts are not considered official. Hand delivered transcript will be accepted as official if they are in a sealed envelope.”

Transcript Cost: $5 for each, $20 for five up to ten at one time.
